Child photo / video consent form
We would be grateful if you would fill in this form to give us permission to take photographs of your child and use these in our printed and online publicity.
By completing this form you:
- give AquaSafe Swim School permission to take photographs and/or video of your child
- grant AquaSafe Swim School full rights to use the images resulting from the photography/video filming for publicity or any other purpose to help achieve the group’s aims. This might include (but is not limited to), the right to use them in their printed and online publicity, social media and press releases.
Conditions of use
- The first name and age of a child along with their club may be used if congratulating an individual child for an achievement but we will NOT include any other personal details without prior permission.
- Copyright of photographs taken will remain with AquaSafe Swim School.
